Submission of Fingerprints

It is highly recommended that a completed fingerprint card be submitted along with your initial appeal request.  A downloadable fingerprint card is available for your convenience.  If your fingerprints are not submitted along with your initial appeal request, you may subsequently be required to submit them in order for the AST to establish positive proof of your identity.
